We are looking forward to meeting you at your upcoming telehealth visit. For many of our patients living outside of the North Vancouver area we offer you the convenience of telehealth visits. Especially with the current COVID-19 pandemic, we are increasingly utilizing telehealth visits to ensure your safety and the safety of our staff.
Preparing for your Telehealth Visit
If you have an upcoming telehealth visit we ask that you are in a quiet and private room during the call.
Please be aware that the phone call may come from an unknown or blocked number.
We make every effort to call you at your scheduled time, but we do ask that you be available to receive your phone call within a 2 hour time window.
If this is your first initial visit please have your list of medications ready and please complete the Health Questionnaire at least one week before your appointment. Please download the form, fill it out (either on electronically or print it out) and return it by fax, mail or drop it off at our office. Download the form here:
What if I can not attend my scheduled appointment?
Your appointment has been reserved for you and we request at least 2 business days notice to reschedule or cancel appointments. Please contact us if you must reschedule or cancel.
No Show Fees:
Full consult – $100 (new or re-referred patients)
Followup – $100
Cystoscopy – $100
Outreach clinics (all appointments) – $100
Surgery – $500
