First Visit and Re-Referrals
If this is your first time coming in to see us, we welcome you and look forward to meeting you. If we have not seen you in more than 6 months, we are happy to see you again.
Please arrive five minutes prior to your scheduled appointment.
Please bring the following to your appointment:
- List of your active medications
- Please complete the Health Questionnaire. Please print the form, fill it out and bring it with you to your appointment. Download the form here:
What should I bring?
Please bring your identification and BC Care Card (Health Card)
Bring in your up-to-date list of medications and allergies.
Please complete your initial visit health questionnaire and bring it in with you to your visit. The questionnaire can be found and printed here.
If you do not speak English, we encourage you to bring a family member or friend who can interpret for you.
What if I can not make my appointment?
Your appointment has been reserved for you and we request at least 2 business days notice to reschedule or cancel appointments. Please contact us if you must reschedule or cancel.
No Show Fees:
Full consult – $100 (new or re-referred patients)
Followup – $100
Cystoscopy – $100
Outreach clinics (all appointments) – $100
Surgery – $500
